We are currently recruiting for a Physical Therapist to join our team!Benefits of this position:Health, Dental and Vision InsuranceTuition ReimbursementPaid Time OffGreat Work Environment401k matched at 10%Flexible Hours (8 hour shifts)Paid HolidaysSame day pay with TapcheckJob location: Beaver Dam, WIPurpose of this positionThe purpose of this position is to evaluate, treat, document and/or facilitate care for residents with impaired ability to perform activities of daily living at an independent level due to disease process; participate in the interdisciplinary team meetings as required; develop restorative nursing programs related to improving residents' function in activities of daily living.Essential Job Duties & ResponsibilitiesEvaluate and document evaluations of referred residents in accordance with facility policies and proceduresProvide appropriate treatment to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilitiesRecommend and/or provide appropriate treatment, preventative measures and adaptive equipment to prevent decline in residents’ functionProvide appropriate resident or caregiver education to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilitiesMaintain timely and accurate resident reports as required by federal, state and local regulatory agencies and third-party payersMake recommendations to administration for adaptive equipment and preventative measures to prevent decline in residents’ functionObserve infection control proceduresBe responsible for the safety of residents under his/her supervision.Assume duties related to completion of Minimum Data Set assessments as assigned.Complete documentation related to evaluations, treatment plans, MDS assessments, resident care plans and progress notes in a timely manner.Participate in resident care conferences, interdisciplinary care conferences and rehabilitation conferences as scheduled.Participate in implementation of programs to improve resident care related to Quality Indicators.Assume Quality Assurance duties as assigned.Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers.Observe all facility policies and procedures.Observe all facility safety policies and procedures.Observe guidelines in the employees' handbook.Follow facility Residents' Rights policies.Adhere to Corporate Compliance Program Code of Conduct and policies and procedures.Attend Corporate Compliance Program in servicing as required.Comply with facility policies and procedures against discrimination and sexual harassment.Protect the privacy of resident Protected Health Information.All other duties as assignedQualificationsTo perform this job successfully, an individual must be able to execute each essential physical and intellectual duty as identified. The requirements listed are representative of the knowledge, skill and/or ability required.Communication/Language Skills — Must have excellent verbal and written communication and organizational skills. Ability to read, analyze, and interpret common medical information. Ability to effectively communicate with residents, their families and other healthcare professionals. Ability to present information to supervisor.Reasoning Ability — Ability to define problems, collect data, establish facts, and draw conclusions.Computer Skills — Must be familiar with software such as Word and Excel.MS in Occupational TherapyNBCOT Certified
