Facility: Skilled Nursing and Rehabilitation Center
Location: Bergen County, New Jersey
Position Type: Full-Time
Salary: $120,000 annually

About the Role

Skilled Nursing and Rehabilitation Center is seeking an experienced Clinical Nurse Manager to oversee the clinical operations of our dialysis program. This leadership position is responsible for ensuring exceptional patient care, regulatory compliance, staff performance, quality improvement, and efficient day-to-day clinical operations.

The ideal candidate is an experienced dialysis RN with strong clinical leadership, regulatory, and staff management skills.

Key Responsibilities

• Develop and implement standards of care in accordance with CMS, ESRD Network, New Jersey State, and Federal regulations , as well as facility policies.

• Oversee all licensed nursing and direct patient care staff.

• Prepare and monitor QAPI statistics , develop action plans, and track performance improvement initiatives.

• Maintain infection control logs and ensure compliance with infection prevention standards.

• Manage EQRS and NHSN reporting and maintain required dialysis documentation.

• Oversee anemia management and monitor patient clinical outcomes.

• Prepare and maintain staff schedules to ensure appropriate coverage.

• Maintain accurate and up-to-date employee files and competency documentation.

• Prepare and maintain Governing Body meeting minutes and related documentation.

• Review water culture results and promptly notify Bio-Medical personnel of any positive cultures.

• Conduct regular staff meetings and communicate clinical and operational updates.

• Review patient plans of care, physician orders, and clinical documentation.

• Monitor medication utilization, equipment logs, and required dialysis records.

• Coordinate staff training, competencies, evaluations, and ongoing education.

• Collaborate closely with the Medical Director, Administrator, and interdisciplinary team.

• Promote patient safety, quality outcomes, and compliance with all applicable regulations.

Qualifications

• Active Registered Nurse (RN) license in the State of New Jersey .

• Minimum of 18 months of dialysis nursing experience .

• At least 12 months of clinical nursing experience .

• At least 12 months of experience providing nursing care to patients receiving maintenance dialysis , including training and experience with the dialysis process.

• Previous Clinical Nurse Manager or dialysis leadership experience required .

• Nursing home/skilled nursing facility experience preferred.

• Strong knowledge of CMS, ESRD Network, Federal, and New Jersey State regulations .

• Experience with QAPI, infection control, quality assurance, and regulatory compliance.

• Experience with EQRS, NHSN, ESRD reporting platforms, and EMR systems preferred.

• Proficiency with Microsoft Office .

• Strong communication, organization, leadership, and problem-solving skills.
