RN Case Manager: Skilled Home Health in Onondaga Region

HCR Home Care

A leading health care provider is looking for a dedicated RN Case Manager in the Village of East Syracuse. In this full-time role, you will provide skilled nursing care to patients, supervise nursing teams, and coordinate patient activities. The ideal candidate should hold an unencumbered RN license and have excellent communication, critical thinking, and organizational skills. This position offers a competitive salary and requires an Associate’s degree in Nursing, with preference given to those holding a Bachelor’s degree. #J-18808-Ljbffr
