Physical Therapist - Outpatient

Assignment Duration

This is a 6-month contract position, starting August 24th.

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. The role includes delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow for an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and self, while adhering to the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on patient discharge.
• Conduct medical record documentation in a clear, concise, accurate, and timely manner.
• Provide care under signed, valid physician orders and ensure compliance with insurance approvals.
• Participate in student programs, providing supervision and evaluation in a positive learning environment.
• Create individualized professional development plans with clinical management.
• Maintain professional licensure in good standing and complete state-mandated continuing education requirements.
• Engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Active CPR Certification.
• Current state licensure for Physical Therapy.
