Your Role & Responsibilities Direct personal care such as assistance with ambulation, eating, grooming, personal hygiene, bathroom assistance including transferring, and medication administration. Completes necessary documentation, including, but not limited to: Therapy data collection, Special Incident Reports, Daily Activity Schedule for clients, Medication Documentation, and other forms of documentation as requested by supervisors. Administers and documents individual medications and provides medical and/or first aid treatment instructed by nursing staff. Protects individual rights, safety, we fare, and health at all times for those we serve. Assures that choices are an integral part of a resident's day and assures that these are available; honors individual choice whenever possible based on safety and individual needs/restrictions. Participates in staff meetings, progress reviews, in-service training, and other meetings as assigned. Delivers individual program plans/goals and behavior support plans as designed; assures active support for all persons served.
