*Job Description & Requirements* Hematology Physician StartDate: ASAP Available Shifts: Day 8;On Call Pay Rate: $388.00 - $420.00 This facility is seeking a Hematology Physician for locum tenens support as they look to fill a current need. Details and requirements for this opportunity: * Schedule: Clinical hours 8a-5:30 pm, plus Call. Call rotation every 4th or 5th week. * Practice Setting: Inpatient/Outpatient/Telehealth * Types of Cases: Trauma level 3, 36-hour patient care/ambulatory clinic coverage per week, Call-Hospital visits Monday-Friday for new consults, Saturday and/or Sunday for rounding only * Credentialing Timeframe: 90 days * Electronic Medical Record (EMR): Cerner * Certifications Required: Board Certified, Fellowship Required, Basic Life Support certifications (BLS) * Licensure Required: Indiana State Medical License or Interstate Medical Compact Licensure *Facility Location* Lafayette is just s short drive north from Indianapolis and just across the river from West Lafayette, home of Purdue University.
