- Assess patient needs and develop comprehensive care plans tailored to individual conditions.
- Collaborate with medical and support staff to coordinate services and resources.
- Monitor patient progress and adjust care plans as necessary to meet clinical goals.
- Facilitate timely discharge arrangements and ensure proper documentation.
- Maintain compliance with hospital policies, regulatory standards, and patient confidentiality.
- Manage complex cases across adult and pediatric populations, ensuring quality care delivery.
- Hold a valid RN license and at least 2 years of acute care experience.
- Demonstrate strong clinical assessment and care coordination skills.
- Possess experience in inpatient care management, preferably with pediatric and adult patients.
- BCLS certification required; additional certifications are a plus.
- Ability to work effectively within a fast-paced hospital environment.
- Must be flexible to float within scope to meet hospital needs.
