Advanced Practice Provider (APP) - Outpatient Palliative Oncology Clinic

Penn Medicine Lancaster General Health is seeking an Advanced Practice Provider (APP) to join our growing Outpatient Palliative Oncology Clinic. This role offers a unique opportunity to partner closely with oncology teams to provide comprehensive supportive and palliative care for patients living with cancer.

The outpatient palliative oncology clinician focuses on improving quality of life through expert symptom management, advance care planning, goals-of-care discussions, and psychosocial support for patients and their families. The APP will work collaboratively with medical oncology, radiation oncology, surgical oncology, and inpatient palliative care teams to ensure seamless continuity of care across the cancer journey.

The APP will manage symptoms commonly associated with cancer and cancer-directed therapies, including pain, fatigue, nausea, anorexia, dyspnea, anxiety, depression, and other complex physical and emotional concerns. In addition, the APP will facilitate discussions regarding patient values, treatment preferences, and future healthcare planning to ensure care remains aligned with individual goals.

This position offers the opportunity to develop longitudinal relationships with patients and families while serving as an integral member of a multidisciplinary oncology care team dedicated to patient-centered, evidence-based care.

Hours

• Full-time, 1.0 FTE
• Monday-Friday
• Day shift hours
• No weekend clinic requirements
• No holiday requirements
• Limited call responsibilities, shared amongst providers

Primary Responsibilities

• Provide outpatient palliative care consultations and follow-up visits for patients with cancer.
• Assess and manage complex symptoms related to cancer and cancer treatment.
• Facilitate goals-of-care conversations and advance care planning.
• Collaborate closely with oncology physicians, nurses, social workers, and supportive care services.
• Partner with inpatient palliative care teams to promote continuity of care during transitions between ambulatory and hospital settings.
• Support patients and caregivers through serious illness using a comprehensive biopsychosocial approach.
• Participate in interdisciplinary care planning and team meetings.
• Assist with development and growth of the outpatient palliative oncology program.

Position Requirements

Nurse Practitioner

• Valid Certified Registered Nurse Practitioner (CRNP) license in Pennsylvania
• Certificate of Prescriptive Authority in Pennsylvania
• Certification by ANCC or other qualifying agency in area of specialty required
• Adult-Gerontology Primary Care or Family Nurse Practitioner certification is required
• Valid Pennsylvania licensure as a Registered Nurse
• Minimum one year of APP experience preferred
• Oncology, palliative care, hospice, or serious illness care experience preferred

Physician Assistant

• Current licensure as a Physician Assistant by the Pennsylvania State Board of Medicine
• Graduate of an accredited Physician Assistant program
• Minimum one year of APP experience preferred
• Oncology, palliative care, hospice, or serious illness care experience preferred

The Outpatient Palliative Oncology Clinic provides specialized supportive care for patients living with cancer. Working alongside oncology colleagues, the team focuses on symptom management, quality of life, decision support, and advance care planning. Through close collaboration with inpatient palliative care services, patients experience coordinated, seamless care across settings, ensuring support throughout diagnosis, treatment, survivorship, and advanced illness.
