POSITION SUMMARY:

The Case Manager works directly with Will Rogers Courts residents to provide personalized, trauma-informed case management support throughout the renovation and relocation process. The Case Manager conducts intake and needs assessments, develops individualized Family Service Plans, connects families to housing, employment, education, and health resources, and monitors progress toward family-identified goals.

DUTIES AND RESPONSIBILITIES:

Program Leadership:

• Conduct one-on-one intake interviews using the standardized Case Intake Form to gather household composition, employment status, health concerns, housing situation, education level, and immediate support needs.
• Administer needs assessment tools, including the Needs Assessment Survey and Risk Evaluation Matrix, to identify priority areas and urgency levels for each family.
• Enter and maintain accurate family data and documentation in ChildPlus (or designated case management database), including individual Family Profiles.
• Provide emergency support referrals for urgent needs such as food security, homelessness prevention, and mental health support.
• Develop individualized Family Service Plans with families, including SMART goals (e.g., securing stable housing, enrolling in workforce training, increasing household income).
• Identify key milestones and coordinate with partner organizations to support families in achieving their goals.
• Conduct regular case reviews (biweekly or monthly) with families to reassess progress and adjust service plans as needed.
• Coordinate service referrals and maintain referral logs across housing agencies, job training centers, healthcare facilities, and educational institutions.
• Organize or connect families to workshops on resume writing, financial planning, mental health awareness, and tenant rights.
• Hold weekly check-ins with assigned families to provide hands-on guidance and support.
• Identify and help resolve barriers and crises (e.g., loss of employment, illness, housing instability) as they arise.
• Track case progress and service logs using the case management database, documenting all family interactions and services accessed.
• Participate in quarterly impact analysis, including family satisfaction surveys and outcome data collection.
• Encourage family participation in community meetings, financial literacy workshops, peer mentorship, and leadership initiatives.
• Support families through the physical renovation and relocation process, providing continuity of care and advocacy.

KNOWLEDGE, SKILLS, AND ABILITIES:

• Ability to build trust and rapport with families experiencing housing instability or transition.
• Strong active listening and crisis-response skills.
• Ability to work collaboratively with community partners and internal team members.
• Cultural competence and sensitivity to the needs of diverse, low-income families.
• Strong written communication skills for documentation and reporting.

WORKING CONDITIONS:

Work is performed in a combination of office and community settings, including resident homes and community meeting spaces. May require occasional evening or weekend availability for community meetings or family emergencies.

QUALIFICATIONS REQUIRED:

• Associate's degree in Social Work, Human Services, Psychology, Counseling, or a related field required; Bachelor's degree preferred. Equivalent combination of education and experience considered.
• Minimum of 2 years of case management, social services, or related direct-service experience.
• Experience or training in trauma-informed care preferred.
• Familiarity with community resources related to housing, employment, healthcare, and education.
• Proficiency with case management databases (e.g., ChildPlus) and standard office software.
• Strong interpersonal, organizational, and problem-solving skills.
• Ability to manage a caseload of approximately 30 families with a high degree of individualized attention.
• Valid driver's license and reliable transportation, as home and community visits are required.
