- Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination
- Interpret and integrate data to determine appropriate treatment plans as needed
- Instruct and counsel patients. Communicate clearly and effectively with patients and other health-care workers
- Collaborate with physicians in managing acute and long-term medical needs of patients
- Order necessary diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
- Interpret diagnostic test results including to assess and identify deviations from normal
- Diagnose and treat acute, chronic, and long-term healthcare issues. Interview and advise patients regarding health and illness prevention
- Recommend resources to meet patient and family needs
- Provide counseling and instruction regarding patient treatment and follow-up care
- Provide effective and meaningful patient education
- Assist in supervision of Medical Assistants and Licensed Practical Nurses
- Effectively identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient care
- Understand and participate in meaningful use data
- Utilize and document in the electronic health record
- Administer emergency care per BCLS, ACLS or PALS standards
- Document medical records with all patient encounters in the office and on the telephone while staying compliant and up-to-date with the EMR process
- Understand and adhere to "incident-to" and "non-incident-to" service and billing guidelines using proper procedures based on service conditions at the time
- Exercise HIPAA confidentiality and security measures at all times during office hours and outside of the office
- Demonstrate responsibility for self-learning through participation in continuing education activities and conferences
- Participate in quality improvement reviews and activities in the clinical setting
- Serve as a clinical resource for staff, clients and families which includes answering triages, help streamline patient flow in office
- Practice as an outpatient provider with thirty-two (32) hours of patient care per week (with availability for 3 patient slots per hour, 96 patient slots per week) and eight (8) hours of administrative time per week, and must be present on-site at least forty (40) hours per week, which includes administrative time. Administrative time must be coordinated and planned to occur outside of patient care hours and when there is otherwise an adequate number of providers to cover overall patient care responsibilities. The particular days scheduled will be worked out with the Medical Director and Practice Manager
- Must have zero charts overdue > 10 days no more than 100 at end of month
- Monitor lab and document box of other providers as assigned and direct any questions or concerns to the Medical Director
- Work some late nights, on average one to two nights per week
- Participate in the outpatient call schedule, which typically includes one weekend per month and the occasional weekday as needed, with physician as backup
- Participate in weekend rotation including up to two Sundays per month, or two Saturdays per month, or any combination of one Saturday and one Sunday per month, at any of Corporation's practice locations; however, additional weekend rotation days could be scheduled from time to time based on operational and clinical needs of the Corporation as determined by Corporation in its sole discretion
- Participate in holiday rotation, working 1-2 holidays per year. When holiday is worked Provider will have 90 days to use as a day off
- Work additional hours/days, as needed, to cover colleague time off and staffing emergencies
- Participate in nursing home on call coverage as needed
- Work in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral
- Timely completes patient care documentation in the outpatient and inpatient settings and required resident evaluations and direct observation assessments as assigned
- Timely completion of Peer Reviews as assigned by Credentialing department to be compliance standards
- Perform other duties as assigned
- Administration of medications and injections, including Tetanus prophylaxis and non-prescription medicine
- Prescribe and dispense relevant to the area of practice for the CRNP
- Prescribe and dispense schedule II with DEA certificate
- Prescribe and dispense schedule III, IV and V with DEA certificate
- Ability to do the following: Cautery; Epistaxis management; Urethral catheter insertion and removal; Collection and preparation of specimens, i.e. gram strains, C&S; EKG tracings; Pap Smears; Foreign body removal: ears, nose, throat, rectum, soft tissue
- Ability to do the following: Visual screening; Corneal screening; Simple removal of foreign bodies; Conjunctiva; Visual Activity
- Ability to do the following: Subcutaneous local and digital anesthesia; Repair simple lacerations; Laceration management; Cleansing/irrigation; Simple edge revisions/suturing; Suture and staple removal; Treatment for minor superficial (1st and 2nd degree) burns; Incision and drainage of abscess; Wound irrigation, packing; Control of external hemorrhage; Carry out aseptic and isolation techniques
- Maintain ongoing appropriate documentation on care coordination to promote awareness and ensure patient safety and follow-through on care plan
- Assist patients in problem solving potential issues related to the health care system, financial and psychological barriers
- Functioning as the system navigator and point-of-contact for high-risk patients and family, with the patient and family having direct access for asking questions and raising concerns.

- Buy in and experience working in the EOS® model (strongly preferred)
- Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride
- Licensed by the State of Pennsylvania as both a Registered Nurse (RN) and a Certified Registered Nurse Practitioner (CRNP)
- Valid DEA Registration Certificate
- Licensed by the State of PA for a CRNP Prescriptive Authority
- Must have completed an accredited, Board-approved masters or post masters nurse practitioner program or other Board approved program that awarded an advanced degree or course of study considered by the Board to be equivalent to that required for certification in PA at the time the course was completed
- Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned.
- Demonstrated knowledge and success in affecting overall clinical operations
- Ability to manage multiple responsibilities and emergency situations successfully
- Proficiency in knowledge of office management, use of computer, software, and office machines
- Familiarity with electronic health records, preferably Medent
- Current BLS, ACLS and PALS certified
- Valid PA driver's license
