Staff Compounding Pharmacist

Position Summary The Staff Compounding Pharmacist at Stratum Pharmacy's Dallas (Coppell) facility is a licensed practicing pharmacist responsible for the verification, clinical oversight, and final release of compounded prescription orders in a 503A pharmacy environment. This role ensures all compounded preparations meet clinical, regulatory, and quality standards in accordance with USP , , , Texas State Board of Pharmacy requirements, and Stratum Pharmacy policies. The Staff Pharmacist provides direct supervisory oversight of pharmacy technicians, supports fulfillment operations, and serves as a clinical resource for staff and prescribers. Key Responsibilities Prescription Verification & Dispensing • Perform final pharmacist verification of all compounded prescriptions prior to dispensing, including therapeutic appropriateness, dosing accuracy, formulation compliance, labeling completeness, and patient safety screening. • Review and approve compounded non-sterile preparations per USP and sterile preparations per USP before release. • Verify BUD assignments are appropriate for the assigned category, container/closure system, storage conditions, and formulation per USP and . • Conduct drug utilization review (DUR) for each prescription: allergy screening, drug-drug interactions, dose range verification, and duplication of therapy. • Authorize prescription release only when all quality, safety, and regulatory requirements are confirmed; document verification per TSBP and facility requirements. • Additional responsibilities as assigned. Clinical Oversight & Prescriber Communication • Serve as the primary clinical resource for prescription clarifications, therapeutic substitutions, and prescriber consultations. • Contact prescribers to resolve incomplete, ambiguous, or clinically inappropriate prescription orders before dispensing. • Provide clinical guidance to technicians and fulfillment staff regarding formulation selection, dosing, route of administration, and patient counseling information. • Document all prescriber communications and clinical interventions in the pharmacy management system. Technician Supervision & Oversight • Provide direct, real-time supervision of pharmacy technician activities in accordance with the Texas Pharmacy Act, ensuring all technician actions remain within permitted scope of practice. • Verify and initial all compounding steps where pharmacist co-signature is required by the master formulation record, batch record, or TSBP requirement. • Identify and address aseptic technique deficiencies, documentation errors, or quality concerns during real-time observation of compounding activities. • Support onboarding, training, and competency development of fulfillment and compounding staff. Quality Assurance & Compliance • Participate in and support quality assurance activities: batch record reviews, deviation investigations, CAPA follow-up, and EM result review. • Review and approve master formulation records for clinical and regulatory accuracy. • Support USP compliance activities for hazardous drug orders: clinical verification, safe handling confirmation, and patient risk-benefit documentation. • Maintain current knowledge of TSBP regulations, USP compounding chapter updates, and applicable federal pharmacy law. • Support TSBP inspections, accreditation audits, and internal quality reviews. Multi-State Licensure & Operational Support • Maintain active pharmacist licensure in Texas plus a minimum of four (4) additional state pharmacist licenses as directed by pharmacy management to support multi-state patient dispensing.Additional state pharmacist licenses as assigned by pharmacy management. • Support fulfillment workflow management and volume distribution to meet daily turnaround and accuracy targets. • Assist with controlled substance log maintenance, DEA reporting, and compliance recordkeeping where applicable. Minimum Qualifications Education • Doctor of Pharmacy (PharmD) or Bachelor's of Pharmacy (RPh) degree from an ACPE-accredited school of pharmacy required. Licensure (Required) • Active, unrestricted Texas Pharmacist License in good standing with the Texas State Board of Pharmacy required prior to employment start. • Must have or be willing to obtain and maintain a minimum of four (4) active state pharmacist licenses as required by pharmacy management for multi-state dispensing operations. • Have completed through a single course, a minimum of 20 hours (ACPE-approved) of instruction and experience in sterile compounding as required by the TX Board of Pharmacy. • Licenses must be maintained in current, active, and unrestricted status; any adverse licensure action must be reported to the PIC and HR immediately. Experience & Knowledge • Minimum 1–2 years of pharmacist experience; compounding pharmacy experience (503A preferred) strongly preferred. • Working knowledge of USP , , and compounding standards and their application in a 503A environment. • Familiarity with TSBP Rule 291.133 and Texas Pharmacy Act requirements for pharmacist practice and technician supervision. • Experience with pharmacy management systems, electronic batch records, and prescription verification workflows. • Strong clinical judgment, attention to detail, and commitment to patient safety. Preferred Qualifications • Experience in a high-volume 503A compounding pharmacy, particularly with sterile, BHRT, topical, or peptide formulations. • Board Certified Pharmacotherapy Specialist (BCPS), Board-Certified Sterile Compounding Pharmacist (BCSCP) or other BPS specialty certification. • PCAB accreditation familiarity or experience supporting compounding pharmacy inspections. • Multi-state pharmacist licensure already active (Texas + 3 or more states). • Experience with controlled substance dispensing programs and DEA compliance recordkeeping. Physical Requirements & Work Environment • Ability to stand, walk, and work at a verification station for extended periods during shift. • Visual acuity sufficient to review labels, prescription documents, and product appearance. • Ability to access cleanroom environments when required, with full gowning compliance per USP . • Work environment: pharmacy fulfillment area, consultation areas, and periodic access to compounding cleanrooms. Supervisory Structure & Oversight The Staff Compounding Pharmacist operates under the general direction of the Pharmacist-in-Charge (PIC) and Fulfillment Manager. Per the Texas Pharmacy Act, each licensed Staff Pharmacist bears personal professional accountability for all prescription verifications, clinical decisions, and dispensing activities they perform. The PIC maintains overall accountability for pharmacy operations; however, pharmacist licensure obligations are individual and non-delegable.
