Seeking a Compassionate Primary Care Physician in Lakewood, CO

Impact to Health has partnered with a mission-driven, privately owned primary care group in Lakewood, CO, to find a Family Medicine or Internal Medicine physician ready to reclaim their clinical autonomy. In this physician-led, value-based organization, patient outcomes-not daily volume metrics-dictate your day.

Redefine Patient Relationships: Step off the fee-for-service treadmill. You will have the time to truly listen to your patients, understand their comprehensive needs, and build the long-term trust required for transformative care.

Practice Highlights

Setting: 100% Outpatient Primary Care
Patient Base: Adult / Senior-Focused / Mixed
Schedule: Monday-Friday
Care Team: Comprehensive support (MAs, Care Coordinators, Admin)
EMR: Athena
Core Focus: Preventive care, chronic disease management, and long-term patient continuity

Requirements
Who We Are Looking For

We are seeking a compassionate, collaborative clinician who thrives in a team-based environment and is deeply committed to patient-centered primary care.

Required Qualifications:

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
Completed residency in Family Medicine or Internal Medicine
Board Certified or Board Eligible (FM or IM)
Active, unrestricted Florida medical license (or ability to obtain licensure prior to start)

The Ideal Candidate Will Also Bring:

A strong interest or prior experience in value-based, care models, or population health
A background working with Medicare or senior-focused populations
A passion for working within an interdisciplinary care team to manage complex chronic diseases
Familiarity with Athena or similar EMR systems
