Job Title: Senior LTSS Service Care Manager (RN) Location: Hybrid – Austin, TX Area Work Arrangement: Hybrid / Field-Based Employment Type: Contract Duration: 6 Months (Potential Extension/Conversion) Pay Rate: $38.00/hr. – $45.00/hr. on W2 Schedule: Monday – Friday | 8:00 AM – 5:00 PM CST (OT may be required) Deadline: 07-18-2026 Skills Required Primary (Must-Have) Active Registered Nurse (RN) License 3+ years of nursing experience 2+ years of case management experience Strong care coordination and service planning skills Experience working with pediatric populations Ability to manage heavy documentation workloads and meet strict turnaround times Strong organizational and time management skills Excellent verbal and written communication skills Ability to work independently in a remote and field-based environment Proficiency with Microsoft Teams and electronic documentation systems Valid Driver’s License and ability to travel throughout the assigned territory Secondary (Good to Have) Home Health experience Pediatric nursing experience Medicaid or Managed Care experience Long-Term Services and Supports (LTSS) experience Community-based care coordination experience Experience working with STAR Kids programs Knowledge of state and federal healthcare regulations Position Overview The Senior LTSS Service Care Manager (RN) is responsible for conducting comprehensive assessments for pediatric Medicaid members enrolled in the STAR Kids program to identify unmet healthcare needs and determine eligibility for Medicaid services and community-based support programs. This role combines remote work with field-based member visits and requires strong assessment, case management, care coordination, and documentation skills. The ideal candidate will possess extensive nursing experience, strong communication abilities, and the capability to manage multiple priorities while delivering quality, member-centered care. Conduct comprehensive in-home assessments for pediatric STAR Kids members Evaluate medical, behavioral, developmental, and social service needs Identify unmet healthcare needs and recommend appropriate Medicaid-funded services Develop individualized care plans based on assessment findings Coordinate services with providers, caregivers, community resources, and interdisciplinary teams Monitor member progress and ensure appropriate service utilization Complete detailed assessment documentation within required timelines Maintain compliance with Medicaid, state, and organizational requirements Ensure documentation meets quality and regulatory standards Meet performance expectations related to documentation accuracy and timeliness Conduct approximately six member visits per week within the assigned territory Coordinate and schedule member appointments Accommodate occasional afternoon appointments extending beyond standard business hours Participate in team meetings, training sessions, and case discussions Collaborate with Team Leads and clinical leadership regarding complex cases Maintain ongoing communication through Microsoft Teams and remote collaboration tools Support company performance goals and assessment completion requirements Perform additional duties as assigned Typical Weekly Schedule Approximately three days conducting member visits (two visits per day) Approximately two days dedicated to documentation and case management activities Heavy documentation workload with strict completion timelines Performance Metrics Include Documentation quality and timeliness Assessment completion requirements Training compliance Company performance goals Minimum Qualifications Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN) Active Registered Nurse (RN) License Minimum 3+ years of nursing experience Minimum 2+ years of case management experience Strong organizational and time management skills Excellent communication and interpersonal abilities Ability to work independently in a remote and field-based setting Valid Driver’s License required Must be authorized to work in the United States without sponsorship now or in the future Preferred Qualifications Home Health experience Pediatric nursing experience Medicaid or Managed Care experience Long-Term Services and Supports (LTSS) experience Community-based care coordination experience Knowledge of STAR Kids programs and healthcare regulations Top Required Skills Exceptional Organizational Skills Strong Time Management Abilities Effective Communication Skills Remote home office-based position Frequent travel to member homes throughout the assigned territory Pediatric member population Collaborative virtual team environment utilizing Microsoft Teams Independent work environment with strong leadership support Compliance Requirements Valid Driver’s License Required Motor Vehicle Record (MVR) Check Required Must be authorized to work in the United States without sponsorship now or in the future Additional Information This is a new position supporting the STAR Kids Program Opportunity for contract extension and potential long-term conversion Candidates must be comfortable with both remote work and community-based member visits Benefits Medical Dental Vision 401(k) Paid Sick Leave EEOC Compliance We are an equal opportunity employer, and all qualified applicants will receive consideration for employment.
