Dialysis Social Work Specialist (MSW)

Fresenius Medical Care is seeking an entry-level Medical Social Worker to support patients with ESRD, dialysis adaptation, and transplant referrals. As part of the interdisciplinary team, you will assess psychosocial needs and coordinate care plans to improve rehabilitation and quality of life.
