Care Manager I IOA is on the forefront of revolutionary healthcare models, reshaping the way people can age in place. With over 23 programs, we offer multiple ways to aid seniors in maintaining their health, well‑being, independence, and participation in the community. Responsibilities Conduct comprehensive assessments and ongoing re‑assessments of residents, including psychosocial, physical, mental health, environmental, and spiritual needs. Write comprehensive assessments and develop client‑centered Community Living Plans based on assessment information. Identify, arrange for, and monitor appropriate community services using knowledge of Medicare, Medi‑Cal, and other entitlement programs. Establish and maintain a care‑management relationship with clients and significant others, offering respect, dignity, and support. Provide crisis intervention, advocacy, problem‑solving, and therapeutic interventions. Meet with clients at least monthly, and more often as needed; review and modify Community Living Plans on an ongoing basis. Document all case‑management activities via progress notes within 24hours, adding new problems to the service plan as needed. Maintain required paperwork and follow a clear, concise, and consistent charting system to allow for continuity of care. Educate clients and significant others about resources and train them to provide their own case management when possible. Maintain open and effective communication with community providers, including physicians and other health‑care and social‑service workers. Monitor the quantity and quality of services provided by other involved providers. Collaborate with the client, caregiver, and involved services to terminate care when appropriate, documenting the process. Participate in research studies and data collection, as required. Participate in and promote ongoing efforts toward continuous quality improvement. Attend and actively participate in IOA and Santa Clara Methodist Retirement Foundation team and program meetings, activities, and problem‑solving endeavors. Use supervision appropriately, maintaining open lines of communication and providing updates on caseload activity. Incorporate the ethical standards of the National Association of Social Workers into all interactions. Understand and apply regulatory and procedural requirements of Community Living Services and Institute on Aging policies. Attend continuing education classes or in‑service training to increase knowledge in case management, gerontology, family and community systems, and related areas. Perform any other reasonably related responsibilities as assigned. Education B.A. or B.S. in Social Work or a related field is required. Background and Experience One year of experience working with disabled adults and/or frail elderly is required. Experience with and understanding of medical and psychosocial problems of functionally impaired adults and the frail elderly is required. Bilingual skills are highly preferred (English plus Cantonese and/or Mandarin and/or Spanish). Exceptional communication and presentation skills related to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals are essential. Demonstrated case‑management skills and experience in the community health‑care delivery system are required. Detail oriented with good problem‑solving skills and the ability to prioritize multiple tasks. Computer literacy is required. Physical & Language Requirements Ability to lift up to 25lbs. Ability to reach, bend, walk, and manipulate five fingers. Ability to speak and write English clearly. Compensation Compensation Range: $73,233 - $87,525/annual. This amount is not necessarily reflective of actual compensation that may be earned and does not include any additional equity, benefits, or other non‑monetary compensation.
