Responsibilities:

• Conduct nutritional assessments and develop personalized dietary plans for residents, taking into account their medical history, dietary needs, and preferences
• Collaborate with the healthcare team to provide comprehensive care, including developing and implementing care plans that address patients' nutritional needs
• Provide education and support to residents and their families on proper nutrition and meal planning, promoting healthy eating habits and lifestyle changes
• Conduct regular rounds to monitor residents' nutritional status and provide feedback to the healthcare team
• Develop and maintain accurate and thorough records of patient interactions, assessments, and treatment plans
• Assist in the development and implementation of policies and procedures related to nutrition care, as well as participate in quality improvement initiatives to optimize patient outcomes
• Participate in committees and other meetings as required, staying up-to-date on the latest industry developments and best practices

Qualifications:

• Bachelor's degree in Nutrition or related field
• Registered Dietitian (RD) certification or eligibility for registration
• Experience working in a post-acute care setting, preferably in a rehabilitation and nursing center
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
• Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
• Strong analytical and problem-solving skills, with the ability to interpret data and make informed decisions
