Role Overview

As a Hospice RN, you will serve not just as a nurse but as a clinical leader, advocate, and reliable guide for patients and their families.

You will oversee the care plan, manage symptoms, and ensure that each patient receives tailored, comfort-oriented care that aligns with their goals and preferences. With each visit, you will provide not only clinical expertise but also reassurance, clarity, and compassion.

Your Responsibilities
  • Lead Patient-Centered Hospice Care
    • Conduct thorough assessments of patients and their families.
    • Create and manage personalized, comfort-focused care plans.
    • Administer skilled nursing care with an emphasis on pain and symptom management.
  • Serve as the Care Coordinator
    • Act as a case manager, coordinating care among the interdisciplinary team.
    • Collaborate with physicians, social workers, chaplains, and aides.
    • Monitor patient conditions and adjust care plans to ensure comfort.
  • Support Patients & Families
    • Provide education, emotional support, and guidance throughout the care journey.
    • Assist families in navigating complex decisions with empathy and clarity.
    • Establish meaningful, trusting relationships during deeply personal moments.
  • Drive Quality & Compliance
    • Maintain accurate and timely documentation.
    • Participate in interdisciplinary team meetings and care planning.
    • Support quality and compliance standards.
Qualifications and Skills
  • Current RN license (or eligibility for compact licensure).
  • Graduate of an accredited nursing program.
  • At least 1 year of clinical nursing experience.
  • Strong clinical judgment and a compassionate approach to care.
  • Ability to independently manage a caseload.
  • Excellent communication skills in sensitive situations.
  • Basic proficiency in electronic medical records (EMR).
  • Current CPR certification.
  • Valid driver's license and reliable transportation.
