Home Health Speech Language Pathologist

Evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities Participate in development and periodic review of Plans of Treatment and Plans of Care Assess and treat disorders of speech, voice, language, hearing, and swallowing Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening Observe, record, and report patient condition and treatment response to supervisors and/or physicians Instruct and train patients in alternative communication systems when appropriate Counsel and instruct patients, families, and healthcare staff Maintain and submit required documentation, including evaluations, visit summaries, care plans, care coordination activities, and progress reports Participate in care coordination and discharge planning Maintain confidentiality and share patient information only when authorized and in the patient's best interests Report to the Clinical Manager Requirements Minimum six months experience as a speech therapist / speech language pathologist Current and unrestricted license Current CPR certification Valid driver's license Auto insurance Reliable transportation Ability to maintain personal vehicle liability insurance in accordance with residing-state minimum limits or specified higher limits Patient-facing role requiring TB screening if selected Home Health experience preferred Core Competencies Demonstrates expertise in evaluating and providing speech/language pathology services, including assessment and treatment of communication disorders, while maintaining compliance with documentation and care coordination standards.
