Hospice CNA

Provide compassionate care to hospice patients, ensuring their comfort and dignity Assist with daily living activities such as bathing, dressing, and grooming Monitor and document patient vital signs and changes in condition using EMR/EHR systems Collaborate with healthcare team members to develop and implement care plans Adhere to HIPAA regulations and maintain confidentiality of patient information Support families by providing emotional support and education about hospice services Drive to patient locations as needed, ensuring timely and reliable service Work Monday through Friday, 8 a.m. to 5 p.m. Serve an assigned territory of usually 1–3 counties near the posted office Requirements 1 year of CNA experience required Licensed CNA in State of Indiana One year hospice experience preferred Current driver's license Reliable transportation and insurance required Core Competencies Demonstrates expertise in providing compassionate hospice care, including daily living assistance and monitoring patient vital signs.
