Onsite Care Coordinator RN — Hospital-to-Home Navigator
Humana
Humana seeks an Onsite Care Coordination Nurse to perform face-to-face assessments for newly admitted members at acute care facilities and to lead discharge planning to ensure safe transitions home. You will identify gaps in care, coordinate with care teams, and educate patients and families to reduce readmissions. The role is hybrid—in-person at facilities with remote documentation as needed—and requires an IL RN license, 3 years of acute care experience, and travel within a reasonable commute. #J-18808-Ljbffr Humana
