Job Summary:
We are seeking a compassionate and patient-focused Primary Care Registered Nurse (RN) to join our ambulatory care team. The RN will provide high-quality, family-centered nursing care in a primary care setting, delivering safe and effective patient care while collaborating with physicians, advanced practice providers, and multidisciplinary teams. The ideal candidate is committed to clinical excellence, patient education, and providing an exceptional patient experience.

Key Responsibilities

• Deliver safe, evidence-based nursing care using the nursing process: assessment, planning, implementation, and evaluation.
• Perform comprehensive patient assessments and recognize changes in patient condition, communicating findings promptly to the healthcare team.
• Collaborate with providers, patients, and families to develop, implement, and update individualized care plans.
• Provide patient and family education regarding treatments, medications, preventive care, and discharge instructions.
• Coordinate care for patients with moderate to complex medical needs while appropriately prioritizing and delegating tasks.
• Administer medications, immunizations, and treatments according to established policies and procedures.
• Maintain accurate, timely, and complete documentation in the electronic medical record (EMR).
• Participate in patient care rounds and contribute recommendations to optimize patient outcomes.
• Promote a positive patient experience through compassionate communication and family-centered care.
• Ensure compliance with hospital policies, infection prevention standards, HIPAA regulations, and quality improvement initiatives.
• Support clinic operations and assist the Site Manager with clinical and administrative responsibilities as needed.
• Participate in unit-based councils, quality improvement projects, and professional development activities.
• Be willing to rotate to other care network locations within the assigned cluster as required.

Required Qualifications

• Graduate of an accredited Nursing Diploma or Associate Degree in Nursing program (BSN preferred).
• Active Registered Nurse (RN) License in the Commonwealth of Pennsylvania or the State of New Jersey, based on assigned work location.
• Current BLS Certification from the American Heart Association (AHA) or American Red Cross with hands-on skills verification.
• Strong clinical assessment, critical thinking, and patient education skills.
• Basic proficiency with Microsoft Office applications and electronic health record (EHR/EMR) systems.
• Knowledge of HIPAA, patient confidentiality, and clinical documentation standards.
• Ability to work collaboratively in a fast-paced ambulatory care environment.

Preferred Qualifications

• Bachelor's Degree in Nursing (BSN).
• Minimum 2 years of nursing experience , preferably in Primary Care, Family Practice, Pediatrics, or Ambulatory Care .
• Previous pediatric nursing experience.
• Prior charge nurse or supervisory experience.
• Membership in a professional nursing organization.
