Clinical Team Specialist

Overview Elara Caring seeks a licensed Occupational Therapist (OT) or Physical Therapist (PT) to serve as a Clinical Team Specialist in Massachusetts (dual licensure in MA and CT preferred but not required). This role supports delivering high-quality home-based care to patients in their homes and aligns with our mission to provide the Right Care, at the Right Time, in the Right Place. Responsibilities Provide oversight of all patient care services, coordinate patient care and referrals, assess patient needs continually, and develop, implement, and update individualized care plans. Review and approve plans of care; evaluate proposed changes for clinical appropriateness. Conduct regular OASIS reviews for all patients. Ensure required visits are completed timely. Review plans of care and related documentation for accuracy and compliance with state and federal requirements. Monitor timeliness of required documentation uploaded to the electronic health record by direct care team members and LPN/LVN Clinical Coordinators; maintain records with accuracy, completeness, and regulatory compliance. Participate in the organization’s strategic planning and QAPI activities. Perform other duties or projects as assigned. Work in a physically demanding, high-stress environment. Travel may be required to support field offices during audits or other events.
