Job Description

We are seeking an experienced Home Health Physical Therapist to provide high-quality care to patients in a variety of clinical settings, including med/surg, cardiac, and ortho cases. The therapist will conduct patient assessments, develop and implement individualized treatment plans, and ensure proper documentation of patient progress.

Responsibilities

• Conduct thorough assessments of patients in their homes.
• Develop comprehensive treatment plans to enhance patient mobility and function.
• Implement effective therapeutic interventions utilizing adaptive devices and modalities.
• Educate patients and families on home exercise programs and safety protocols.
• Document patient progress and outcomes in compliance with regulations.
• Collaborate with interdisciplinary teams to optimize patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in home health care.
• Valid state licensure as a Physical Therapist is mandatory.
• Current BLS certification required.
• OASIS training preferred.
• Experience with Medicare, Medicare Advantage, and commercial insurance documentation.
