Registered Nurse - Home Health

Job Description

Job Summary:

Provide comprehensive home health nursing care to patients in their residences, ensuring adherence to care plans and safety protocols. Collaborate with interdisciplinary teams to promote patient health and recovery.

Responsibilities:

Assess patients' health conditions and develop individualized care plans.
Administer medications and treatments following physician orders.
Educate patients and caregivers on disease management and health maintenance.
Document patient progress and update medical records accurately.
Monitor and evaluate patient responses to interventions and modify care plans as needed.
Coordinate with physicians and other healthcare professionals to ensure quality care delivery.

Qualifications:

Valid Registered Nurse (RN) license in the state of practice.
Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an approved vendor.
Valid driver's license and reliable automobile with current automobile insurance.
Minimum age of 21 years or older.
Demonstrated experience in home health nursing preferred.
Strong communication and patient education skills.
