Licensed Practical Nurse

Job Description

The Licensed Practical Nurse (LPN) plays a vital role in providing comprehensive care within a long-term care facility. Working under the direct supervision of an RN Supervisor and as part of a multidisciplinary care team, the LPN is responsible for delivering high-quality patient care through clinical tasks and attentive collaboration. The role requires proficiency in specific electronic health records (EHR) systems for accurate documentation.

Responsibilities

Provide direct patient care tailored to individual needs
Administer medications safely and accurately
Monitor and record vital signs consistently
Perform wound care procedures and manage dressing changes
Maintain detailed and compliant documentation using EHR software
Collaborate effectively with the multidisciplinary care team

Required Qualifications

Licensed Practical Nurse (LPN) diploma or certificate from an accredited program

Preferred Qualifications

Entry-level experience in licensed practical nursing
