As a Wellness Nurse (LPN/LVN), you support resident health, monitor care needs and ensure safe medication practices and clinical compliance.What You'll Do:Conduct wellness assessments and monitor resident health changesSupport medication management and perform MAR/TAR auditsCommunicate with families, physicians and care teamsMonitor incidents, falls and follow-up careMaintain documentation and compliance with regulationsSupport infection control and clinical quality initiativesTrain and support care team membersWhat You'll Bring:Active LPN/LVN license1+ year experience in senior living or clinical settingStrong assessment and documentation skillsKnowledge of medication administration and complianceStrong communication and problem-solving skillsPay Range: 33.30 - 44.50Ready to take the next step and make a bigger impact than you ever imagined?
