Home Based Primary Care Assistant Program Director-Registered Nurse

Job Title

Major duties and responsibilities include, but are not limited to:

Oversee the management, supervision and evaluation of care for patients enrolled in the HBPC programs that are organizationally aligned under HBPC.
Leading visioning, strategic planning, staffing, direction, budgeting, evaluation, and tracking of HBPC programs, while fostering strong relationships with VA and community-based partners.
Responsible for developing, organizing, guiding, controlling, and evaluating program structures and outcomes; resolving complex personnel management challenges; participating in executive-level decision-making; and providing leadership in implementing standards for interdisciplinary practice.
Ensure allocation of appropriate resources and prioritizing staff safety and well-being within the HBPC Program.
Advance the development and effective functioning of interdisciplinary health care teams, recognize and optimize the unique contributions of each team member.

Work Schedule: Monday-Friday 8:00am-4:30pm

Telework: May be authorized on an ad-hoc basis
