Per Diem/Part Time Hospice MSW Social Worker

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. Responsibilities Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal‑directed care plans. Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family. Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record. Conduct reassessments of patient and family needs and provide counseling as required. Provide crisis intervention and individual or family counseling when indicated. Serve as a care advocate for the patient and family unit. Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals. Provide ongoing counseling related to issues of death and dying to the patient and family as needed. Participate as a member of the Bereavement Team as assigned. Adhere to all hospice policies.
