Description
Klarus Home Healthis seeking compassionate and dedicatedRegistered Nurses (RN) Case Managerto join our team. We are looking forfull time, part time, PRN / Per Diem RNswho are passionate about providing qualityhome health careto our community-based patients inAurora, Centennial, Glendale, Englewoodand the surrounding areas.

Requirements
Current State or Compact Registered Nurse (RN) License in hand
A minimum of one-year clinical experience is preferred
OASIS knowledge highly preferred
Demonstrate knowledge and skillset in current nursing practice
Possess a valid state driver's license and automobile liability insurance
HCHB EMR knowledge is desired
Follow a designated care plan in accordance with patient's needs
Make home visits to clients in designated geographic territories
Perform assigned duties, including administration of medication, wound care, treatments, and procedures
Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments
Case management and coordination
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
