Home Infusion Therapy Registered Nurse (RN)

Venture Forthe is seeking a Home Infusion Therapy Registered Nurse (RN) to provide high-quality infusion therapy services to patients in the comfort of their homes. This role combines hands-on clinical care, patient education, care coordination, and professional collaboration in a supportive and mission-driven environment. This position will serve patients in Boome, Cortland Delaware, Tompkins, Onondaga Counites.

What You'll Do

• Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
• Assess patients and develop individualized infusion plans of care.
• Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
• Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
• Monitor patients for infusion reactions, side effects, and clinical changes.
• Maintain accurate and timely clinical documentation.
• Support quality improvement initiatives and ongoing competency programs.
• Assist with training and supervision of paraprofessional and nursing staff.

Work Environment

This position involves travel to patient homes and community settings throughout the service area. Travel is reimbursed. The role may require lifting and moving equipment and supplies in accordance with job requirements.

Requirements

What We're Looking For

• Current New York State Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• At least 1 year of acute care nursing experience preferred.
• Infusion therapy experience strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
• Home care experience preferred but not required.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently in varied home environments.

Salary Description $40 - $45/hour
