Job Description Home Health Occupational Therapist (OT) Coverage Areas: North Palm Beach County, South Palm Beach County, Central Palm Beach County and surrounding area. Job Summary: Occupational Therapist (OT) will provide coordinated care to clients of all age groups. Plans implements and evaluates client care plans to restore or maintain patient well-being. Perform clinical assessments; identifies patient needs and appropriateness of client. Document, prepare/revise, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). Assess and facilitate through direct therapeutic intervention clients' ROM, strength, coordination need for and use of adaptive equipment and environmental modifications, functional mobility, and safety issues as related to clients' ADL performance needs. Provide effective communication to clients, their families, team members, and other health care professionals, and participate regularly in case conferences/team meetings. Facilitate the use of these skills by other team members to achieve desired outcomes. Evaluate client's occupational performance in the areas of self‑care, work, leisure, and rest. Assess sensory‑motor neuromuscular, psycho‑social, and cognitive‑perceptual components of occupational behavior. Monitor assigned cases to ensure compliance with the requirements of third‑party payers. Complete appropriate documentation in a timely manner to assure compliance with company policy. Demonstrate commitment and professional growth by participating in in‑service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the Agency/licensing board. #FLHIGH Requirements: Experience: Two (2) years’ experience as an OT in a clinical care setting. Preferred, home health experience but not necessary. Skills: Therapy skills as defined by generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision‑making skills. If required to make visits, Hepatitis profile. Must read, write and comprehend English. Education: Bachelor of Science in Occupational Therapy. Licensure/Certification: Current driver’s license in good standing. It is the employee’s responsibility to renew the license before it expires in order to continue employment. Reliable transportation and auto liability insurance. Current State professional license as an OT in any/all applicable states. Current CPR. On‑going employment as an OT always requires maintenance of a valid OT license in the state where the therapist practices and a valid CPR certificate; renewal before expiry is required for continued employment. Physical Requirements: Prolonged standing/walking required. Ability to lift/transfer up to 50 lbs. Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions. Meet patient/family individualized psychosocial needs. Requires hand‑eye coordination and manual dexterity. Environmental/Working Conditions: Works in patient’s home in various conditions. Possible exposure to blood‑borne pathogens, bodily fluids and infectious diseases. Some exposure to unpleasant weather; PRN emergency call.
