Home Health AideUnited Health Services (UHS) is seeking a compassionate and dependable Home Health Aide to join our home care team. In this role, you'll provide personal care and assist with daily activities and rehabilitative exercises, helping patients live safely and comfortably in their own homes. Our aides benefit from flexible schedules, strong clinical partnerships, and the chance to develop meaningful relationships with patients and families over time.Job Responsibilities:Provides hands-on personal care and assistance with activities of daily living, such as bathing, dressing, grooming, and toileting, in accordance with the patient's individualized plan of careDelivers care under the supervision of a Registered Nurse, following New York State Department of Health guidelines and all agency policies and proceduresEncourages and supports patients in performing tasks independently to promote self-care and maintain dignityAssists with light housekeeping tasks to ensure a clean, safe, and organized home environment that supports the patient's well-beingPrepares and serves meals using basic nutrition principles and modifies diet plans as directed by the Registered Nurse to meet the patient's health needsProvides care using infection prevention practices and reinforces infection control education given by nursing staffAids patients with prescribed rehabilitative exercises under the guidance of a Rehabilitation Therapist to support recovery and mobilityObserves the patient's physical, emotional, and behavioral condition and accurately records daily care activities and any significant changesCommunicates changes in the patient's condition immediately to the Registered Nurse or, if unavailable, to the Home Health Aide SupervisorParticipates as a valued member of the care team, offering feedback and suggestions to improve the quality and continuity of patient careCompletes all documentation in a timely, thorough, and accurate manner according to agency standards and regulatory requirementsProvides compassionate, respectful, and culturally sensitive care while maintaining patient privacy, dignity, and comfort in the home settingPosition Qualifications:High School Diploma or GED and successful completion of a Home Health Aide program within 12 months of hire, ORCurrent Home Health Aide Certification (no diploma/GED required if you're currently certified)Why You'll Love Working at UHSAt United Health Services (UHS), we believe every connection—to patients, to purpose, to each other—makes a difference.
