Clinical Referral Coordinator

Role Overview This is a full-time position requiring the coordination of home care services, primarily at a hospital location, with flexibility to travel between sites. The role involves acting as a liaison between patients, physicians, hospital staff, and home health agencies, supporting discharge planning, educating staff and community on home care options, and participating in sales and marketing efforts.
