Key Responsibilities

• Conduct thorough assessments and accurate diagnoses for residents experiencing acute and chronic health conditions.
• Develop and execute individualized evidence-based treatment plans in collaboration with physicians and other team members to effectively address medical and social needs.
• Order, perform, and interpret necessary diagnostic tests while prescribing appropriate medications as part of a comprehensive care strategy.
• Facilitate health education for residents and their families regarding chronic disease management, medication usage, and post-discharge care procedures.
• Engage in interdisciplinary rounds, collaborating with healthcare professionals to collectively monitor patient progress and optimize outcomes.
• Maintain up-to-date and detailed electronic medical records using established healthcare information systems to ensure patient safety and care continuity.
• Contribute actively to quality improvement initiatives, ensuring compliance with evolving healthcare standards and evidence-based guidelines.
• Stay informed of advancements in nursing practices and regulatory changes to ensure the highest standard of care.

Patient Population / Client Focus

Our Assisted Living Facility primarily serves elderly residents, many of whom manage complex multi-morbidities, including cardiovascular issues, diabetes, and cognitive impairments. As the Senior Primary Nurse Practitioner, you will play a pivotal role in crafting tailored healthcare solutions that support our residents' physical, emotional, and social well-being.

Qualifications

• Current certification as a Nurse Practitioner (NP) is mandatory.
• An active nursing license in the state of Maryland is required.
• A Bachelor’s degree in Nursing from an accredited institution is essential, while a Master's degree in Nursing is strongly preferred.
• Demonstrated clinical experience, ideally a minimum of 3-5 years, working in an assisted living or similar healthcare setting.

Compensation and Benefits
