RN - Weekend RN - Home Care Oak Creek

Position Summary Weekend Visit Nurses (Baylor RN) at Horizon Home Care & Hospice. Provide quality home care to patients, coordinate services, and ensure standards of practice. Responsibilities Assess patient conditions and family/caregiver conditions accurately and completely during each visit. Identify and provide for patient and family’s educational needs. Provide patient care per physician orders and agency policy. Complete all required patient and agency paperwork accurately, thoroughly, legibly, and timely. Transcribe, implement and evaluate new physician orders accurately. Communicate patient issues and concerns to the Care Manager, Patient Care Coordinator, appropriate health professional, IDG (Hospice only), and/or Clinical Manager in a timely, accurate manner. Perform and document admissions in a timely, thorough, and accurate manner. Qualifications Degree from an accredited School of Nursing (BSN preferred). Current WI RN license. 1-2 years of Med/Surg or Home Care experience. Current/Valid WI Driver’s License and reliable/insured vehicle. Benefits Up to $9,000 hiring bonus.
