Actions are consistent with hospital policies and procedures and current standards/regulation governing nursing practice and long term care.MINIMUM QUALIFICATIONS :EDUCATION, CERTIFICATION, AND/OR LICENSURE:1. High School diploma or equivalent.2. Current Certified Nursing Assistant Certification through the West Virginia DHHR.3. CPR certification.PREFERRED QUALIFICATIONS :EXPERIENCE:1. Collects and prepares specimens according to policy and procedure including, but not limited to urine, stool, and/or sputum.7. Administers care and treatments for patients as instructed by the Registered Nurse, including but not limited to:Weighs patientGives enemaProvide activities of daily living/restorative carePerforms perineal carePerforms catheter carePerforms oral hygieneApplies hot or cold compressesStrains urineObtains T.P.R.,temperature, pulse and respirationsObtains intake and outputAmbulates patientsTurns and positions patients maintaining proper body alignmentProvides sitz bathProvides pre‑operative careProvides post‑operative carePerforms post‑mortum careApplies assistive devices, braces and equipmentRestraint application and the identifications of restraint alternatives.8. Performs basic routine duties including, but not limited to:Distributes fresh water every shiftDisposes of soiled linen every shiftMaintains clean and orderly unitMaintains and stocks unit suppliesPractices clean and sterile technique in the delivery of patient care while maintaining body substance isolation.Aware of the location of equipment and supplies on the nursing unit.9. Demonstrates knowledge and skill necessary to provide care and education appropriate to the age of the patients served. (Adult 18-79 years, Geriatric 80+ years)Performs food and nutritional duties including:Distribution of snacks and supplementsMaintains fluid restrictionsAssists patients with meals and supplements as needed.Documentation in the electronic medical recordProvides care for the respite and hospice patient.10. Attends in-services and other educational programs as required with emphasis on programs related to the geriatric patient.11. Attends and participates actively in a minimum of three staff meetings annually.12. Demonstrates proficiency, competence and current knowledge in providing patient care.13. Determines areas of self-improvement14. Supports and adheres to hospital, nursing service and skilled care policies and procedures.Provides restorative patient care in a manner that promotes maintenance or enhancement of the patient’s quality of life.Participates in continuous improvement activities.Sets short term goals to organize work to meet patients needs.Institutes emergency measures calmly and effectively.Prepares and submits incident reports.Reports safety or risk management problems to Director and charge nurse or appropriate hospital personnel.Supports hospital and departmental cost containment efforts.Reviews and initials meeting minutes and postings in the department.Responds to patient’s call light immediately; notifies nurse of patient problems.Reports observations, requests and unusual signs and symptoms of patient to charge nurse or team leader immediately.Keeps Director informed about patient care issues and departmental activities.Assists with the implementation of the Activities program as needed.Provides planned individual and group activities or provides activity of patient’s choiceEnsures patient’s bedside calendar updated and/or replacedTransports patients to and from activitiesNotifies activity personnel of pertinent patient care information (i.e. fluid restrictions, isolation, etc.)16. Provides the appropriate educational information to the patient and his/her significant other throughout stay under the direction of a registered nurse.17.
