Responsibilities
- Perform initial PT evaluations for residents in the long-term care setting and develop individualized treatment plans in collaboration with the interdisciplinary team.
- Deliver evidence-based therapies addressing mobility, gait and balance, strength, range of motion, and activities of daily living to restore or maintain function.
- Monitor patient progress, document outcomes in the medical record, and adjust treatment plans as clinically indicated.
- Ensure patient and staff safety during therapy sessions, adhere to infection control standards, and follow facility policies.
- Educate patients and families on home exercise programs and activity modifications to support ongoing independence.
- Collaborate with physical therapy aides and other disciplines to optimize rehabilitation outcomes and coordinate care transitions.

Requirements
- Active Physical Therapist license in Montana or eligibility for MT licensure; in good standing
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work effectively in a long-term care setting with older adults
- Experience in physical therapy; long-term care setting preferred
- Strong communication and teamwork skills; able to document clearly and coordinate with care staff
