What You'll Do You'll be an important part of helping patients move from hospital care to the next stage of recovery. In this role, you will: Help arrange home health services and medical equipment needed after discharge. Coordinate follow-up appointments with primary care providers and specialists. Assist patients who do not currently have a primary care provider in establishing care. Support arrangements for outpatient therapy and other post-hospital services. Provide discharge planning support in partnership with RN Care Managers and Social Workers. Document care coordination activities accurately in the electronic medical record. Deliver Medicare Important Message letters and verify patient demographic information. Communicate with clinics, providers, insurers, and community partners to support patient care transitions. Welcome visitors, answer phone calls, and connect people to the appropriate team members. Support department operations through scheduling, reporting, data entry, and other administrative tasks. Help ensure patients, families, and care teams receive timely information and support. What You'll Bring Required High School Diploma, GED, or equivalent. Three years of administrative experience in a professional office setting. Strong communication and customer service skills.
