PRN Home Health Physical Therapist Assistant

HCA Healthcare is seeking a PRN Home Health Physical Therapist Assistant to join our Healthcare & Medical Services team, supporting patients in their homes with high-quality, compassionate rehabilitative care. In this role, you will work under the supervision and direction of a licensed Physical Therapist to help patients regain mobility, improve function, and safely remain in their home environment following illness, injury, surgery, or exacerbation of a chronic condition.

As a PRN Home Health Physical Therapist Assistant, you will provide one-on-one, hands-on care in patient homes, implementing individualized treatment plans that may include therapeutic exercises, gait and balance training, transfer training, endurance activities, and functional mobility tasks. You will closely observe patients’ performance and tolerance, accurately measure progress, and update the supervising Physical Therapist and interdisciplinary team on patient response and any changes in condition. You will play a key role in educating patients and caregivers on home exercise programs, safe use of assistive devices, fall-prevention strategies, and techniques that promote independence with daily activities.

This position requires strong clinical judgment within the PTA scope of practice, excellent communication, and the ability to work independently in a variety of home environments. You will complete timely and thorough electronic documentation in accordance with HCA Healthcare policies, payer requirements, and regulatory standards. You will collaborate with nurses, physicians, social workers, and therapists to support coordinated, patient-centered care and may participate in case conferences and team meetings.
