Hospice Liaison

Allied

Allied Services seeks a Hospice Liaison RN to develop referrals and facilitate admissions for inpatient hospice, home hospice, and community palliative referrals. You will evaluate referrals, complete necessary paperwork, and market services to referral sources while ensuring age-specific needs are met within Allied Services’ care levels. The role requires an accredited nursing degree, an active PA RN license, and strong communication skills; travel to referrals sources may be required. #J-18808-Ljbffr
