Bluegrass Care Navigators seeks a Home-Based Primary Care Physician to deliver in-home care to patients in their homes and care facilities. You will supervise PCP nurse practitioners, guide clinical practice, and coordinate care for chronic and urgent conditions.
Responsibilities include evaluating illnesses, ordering tests, referrals, prescribing medications, and performing procedures within your scope. Kentucky licensure and DEA registration are required.
