Home Health Care Services Coordinator

To coordinate and ensure quality and safe delivery of home health care services. To Coordinate provision of home health care services that reflect the Agency Philosophy and standards of home health and nursing care of assigned patients. To ensure quality and safe delivery of nursing care services.

Licenses & Certifications

Qualifications

*Ability to communicate with a wide range of people to create a positive image of Tanner Health System *Basic computer skills required*One year experience as an RN II at Tanner or a minimum of 24 months experience as a Registered Nurse for new hires*Pleasant, professional demeanor, confident personality*Proven ability to make independent judgments, displaying emotional maturity and using sound judgment*Proven ability to organize and prioritize for maximum time utilization*Proven ability to work with others at all levels within and outside Tanner

Area of Responsibilities

*Make home visits on patients for the purpose of Admissions, Post-hospital, Recertification, skilled nursing visits, Aide supervisory visits and/or discharge visits, as assigned.*Meets or exceeds productivity goals in accordance with Agency established 70th percentile. *Proficient in use of Maestro software to navigate patient record, enter orders, initiate and implement plan of care, and complete Agency required documentation for all visit types within 48 hour time period.*Provides ongoing communication with appropriate staff members to make known any changes in patient needs, schedules, physician orders, special instructions and patient updates.*Verifies the Plan of Care prior to each visit and provides care according to physician's orders and Agency policies and procedures.*Adheres to all Infection Control guidelines in accordance with Agency policies and procedures.*Appropriately updates/revises Medication Profiles and Home Health Aide care plans as assigned per agency policy.*Assesses, plans, implements and evaluates patients on every visit and documents in compliance with Agency policy/procedure.*Assist in ensuring that the patient home folder is kept accurate and current.*Assists in orientation of new RN/LPN staff members and preceptor to nursing students, as requested.*Attends Reassessment/Case Conferences regarding patients and provides input for care planning and reassessment.*Charges for any equipment and supplies for patient care are documented for appropriate reimbursement.*Communicates any changes in a patient's status and/or Plan of Care to the Client Care Supervisor.*Compares/verifies actual care given (as ascertained on Aide supervisory visit) by Home Health Aide to current Home Health Aide Care Plan, documents visit on supervisory note and reports any discrepancies.*Completes any QA requests within 48hours of receipt.*Demonstrates maintenance of confidentiality of patient information.*Develops and implements discharge plans and provides appropriate education and discharge instructions to patients, upon discharge, in accordance with Agency policy and procedure.*Ensures that all clinical notes, supply/visit logs, etc. are accurate and submitted per Agency policy.*Implements and documents appropriate nursing interventions to meet patient needs and changed conditions.*Must perform intermittent visits on patients receiving care from LPN's, as assigned.*Participates and maintains the Agency philosophy, mission statement and administrative policies and procedures.*Participation in in-services and/or continuing education programs is consistently demonstrated.*Prepare appropriate documentation for mileage, visits and/or time and submits to the Client Care Supervisor on a timely basis.*Provides nursing care requiring specialized skills, per current nursing practice and Agency policies and procedures.*Remains available and is able to be located during all assigned working or 'on call' hours.*Rotates 'on-call' as assigned for weekends and holidays.*Writes verbal orders for initial and any changes to patient Plan of Care (including PRN visits) on the day of the identified change and/or visit.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Supervision

*Performs Home Health Aide supervisory visits every 14 days and LPN supervisory visits every 30 days.

Requires frequent contact with many persons at different levels inside and outside of the organization to carry out organization policies and programs and obtain willing acceptance, consent, or action.

Effect Of Error

Probable errors not easily detected and may adversely affect external as well as internal relationships and may result in major expenditures for equipment, materials, or procedures detrimental to the patient's welfare or the organization's interest. Work is subject to general review only and requires considerable accuracy and responsibility. Continually works with reports, records, plans, and programs of a major functional area of the organization where integrity is required to safeguard the organization's position. Duties may involve the preparation of data on which the administration bases important decisions and are highly confidential.

Supervisory Responsibility

Has limited supervision over a small section of employees (up to 15), i.e. assigns and directs their work; instructs new employees and corrects results, may discuss with supervisor, but has no authority over job content or personnel. Team governance where the Members evaluate, interview, and recommend personnel actions.

Mental Demands

A wide variety of complex, changing problems, most of which cannot be anticipated, and there is little existing precedent. Requires careful analysis for the effect of solution on other activities and overall coordination in the organization. Accuracy is essential and not subject to further check. Work involves high degree of resourcefulness, independent judgment, initiative, and long-range planning to achieve major functional objectives.

Physical Effort

Medium physical effort - Lifts, carries, or handles lightweight (1 to 25 lbs.) materials or equipment for most of the day. Occasional physical effort with medium weight objects (25-60 lbs.). Very occasional physical effort with heavy objects (over 60 lbs.).Works in reaching or strained positions for about half of day.

Working Conditions

Considerable - (About 75% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk and probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
