Memory Care Coordinator

We’re looking for a motivated Memory Care Coordinator to join our team! This position assists the Resident Services Director and/or the Health Services Director with monitoring resident health, including: Record Keeping Scheduling of Personal Care Assistants, developing daily tasks and making assignments Self‑Medication Monitoring and Record Keeping Initiate and Maintain Emergency Packets Organization of Resident Charts Job Responsibilities Medication management: ensure that medications are properly stored and cared for. Observe residents' health status, take actions to address concerns with scope of practice, record changes, and notify Resident Services Director. Lift/transfer residents as needed. Maintain residents’ records daily in a timely manner and in accordance with company policy and procedures (includes documentation, med distribution, leisure activities, incidents and observations, charting any changes in physical conditions or behavior). Schedule Personal Care Assistants and develop daily tasks and assignments. Evaluate resident care needs and determine the required level of care. Requirements High school diploma or equivalent certificate. Valid First Aid and CPR Certification required. Must possess a certified nursing assistant certification. Must have a reliable source of transportation. Must have a team‑player mindset with attention to details. Timely and organized. Able to speak, read, write and understand the English language fluently. Able to pass a background check and drug screen. Work Schedule Tues­days – Saturdays (schedule may vary). Why join us?
