Job Summary:

Seeking a licensed Physical Therapist to provide home health services in Solano County, California. The role involves prioritizing start of care visits, supervising Physical Therapy Assistants, and ensuring timely documentation and communication.

Responsibilities:

• 
  Prioritize and execute start of care visits for new patients.
  
  
• 
  Supervise and mentor Physical Therapy Assistants during patient care.
  
  
• 
  Complete clinical notes and other documentation within 48 hours of each visit.
  
  
• 
  Communicate promptly with supervisors, intake, and office staff regarding patient status and scheduling.
  
  
• 
  Maintain compliance with all state licensing requirements and company policies.
  
  

Qualifications:

• 
  Hold a valid California Physical Therapist license.
  
  
• 
  Possess at least one year of Home Health experience.
  
  
• 
  Demonstrate strong organizational and communication skills.
  
  
• 
  Be capable of working independently and as part of a multidisciplinary team.
  
  
• 
  Maintain reliability and responsiveness during standard business hours.
