Senior Registered Nurse and Clinical Team Lead

Responsibilities Conduct nursing assessments; obtain, record, and update individuals’ health information and vital signs using standard instruments and procedures. Prepare, implement, and modify nursing care plans; assist with examinations and treatments; administer medications and nursing procedures as prescribed. Observe, monitor, and record individuals’ symptoms, conditions, and care; identify and report medication responses or changes in health status. Serve as the nursing representative on a treatment team; provide information on nursing needs and consult and coordinate with treatment teams and other health care disciplines to assess, plan, implement, or evaluate care. Make clinical rounds to assess nursing care needs and observe and participate in the provision of nursing services. Provide health teaching, counseling, and education to individuals, families, and other groups; develop health improvement programs. Implement infection control protocols and procedures; provide related consultation and instruction to staff. Maintain and update charts, records, and other documentation, consistent with agency, State, and federal requirements; may collect data and perform related activities to support medical research. Monitor inventories and ensure standards for medications, solutions, supplies, and equipment are maintained. Supervisory Responsibilities Provide clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff. Delegate nursing activities, skills, or procedures to staff; make clinical rounds to assess needs and evaluate the provision of nursing services. Act as liaison to outside agencies and providers to ensure continuity of care, advocate for individuals, and advise on discharge plans. Perform supervisory duties over lower‑level nursing staff; provide information to Registered Nurse Supervisors and contribute to reports and performance evaluations. Minimum Qualifications Must hold an active license and current registration as a registered professional nurse in New York State.
