Job Summary

Empath Home Health, a member of Empath Health, is seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team in Sarasota, FL .

As a Home Health Physical Therapist, you'll help patients regain and maintain their highest level of physical function while delivering personalized care in the comfort of their homes. You'll work alongside a collaborative clinical team dedicated to improving patients' quality of life through Empath Health's Full Life Care philosophy.

Key Responsibilities

• Provide skilled physical therapy services as prescribed by a physician.
• Perform comprehensive initial and ongoing patient assessments, including OASIS assessments .
• Evaluate patients' functional abilities and activities of daily living (ADLs).
• Develop, implement, and revise individualized treatment plans in collaboration with physicians and the care team.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate changes in patient condition and treatment response to the physician and appropriate care team members.
• Ensure documentation is accurate, timely, and compliant with agency policies and state/federal regulations.
• Educate patients and caregivers on treatment plans, exercises, and strategies to improve mobility and independence.

Minimum Qualifications

• Doctor of Physical Therapy (DPT) from a CAPTE-accredited program
• Active Florida Physical Therapist License
• Current BLS Certification
  (American Heart Association or American Red Cross)
• One to two (1–2) years of Physical Therapist experience
