About the Role:

Family & Friends Homecare is seeking a detail-oriented, compassionate Care Coordinator who is fluent in both English and Spanish to support our clients, caregivers, and Case Managers/Referral Sources. This role is critical to ensuring services are delivered smoothly, communication is clear, and families feel supported throughout their care journey.

Key Responsibilities:

• Serve as a primary point of contact for clients, families, caregivers, and referral partners

• Meet with potential clients and their families

• Perform intake visit with New Clients

• Create Care Plans for HCBS and IHSS Medicaid Clients

• Communicate with Case Managers regarding Client Care Plans

• Understand Medicaid Eligibility and PARs

• Schedule and Complete Quarterly Supervisory Visits with Clients

• Assist with the Incident/Complaint Reporting process, following up on incidents or complaints and ensuring proper and timely communication

• Maintain accurate client records

Qualifications:

• Fluent in English and Spanish (required)

• Previous experience in care coordination, home care

• Strong organizational and time-management skills

• Excellent communication and customer service skills

• Comfortable using software, email, and basic office systems

• Ability to multitask in a fast-paced environment

• Knowledge of Medicaid, home care
