Clinician I - TAY (Regular Full-Time, Hourly) Program: Heartland Wellness & Recovery Center (El Cajon) Region: El Cajon Services: Adult Outpatient (AOP) Career Type: Clinicians Hours 40 hours per week. Monday, Tuesday, Thursday, Friday, 8:30 am to 5:00 pm and Wednesday, 10:30 am - 7:00 pm. Conduct initial behavioral health assessments for clients, utilizing a psychosocial rehabilitation focus and dynamic team-based approach. Conduct individual and group counseling sessions. Develop and update individual treatment service plans for clients. Create discharge plans and provide linkage to other support services within the community. Coordinate outreach for TAY clients and TAY specific providers in the community. Participate in interdisciplinary treatment planning. Provide crisis intervention services as needed. Additional Qualifications and Skills Experience working with a transitional aged youth mental health population, especially individuals with severe and persistent mental illness and co-occurring disorders strongly preferred Must be able to work in a fast-paced and collaborative environment Able to pass criminal/background clearance #J-18808-Ljbffr CRF Behavioral Healthcare
