Cares Assessor - 65000435
Community and Social Services
The CARES Assessor conducts comprehensive assessments of applicants for nursing facility placement and home and community-based services for the Department of Elder Affairs, evaluating Level of Care and recommending appropriate placements. The role involves coordinating with interdisciplinary teams, maintaining accurate client data in the CIRTS system, and ensuring compliance with State and Federal regulations governing long-term care services. This is an on-site position requiring travel within the assigned planning and service area. Key qualifications: Four or more years of professional experience in community and social services, or a bachelor's degree in the field, or equivalent combination of education and experience Knowledge of Level of Care rules, regulations, CARES policies and procedures, and aging process Strong written and oral communication skills with ability to compile, organize, and analyze client information Proficiency with Microsoft Windows and Office Suite products (Word, Excel, Outlook, OneDrive, SharePoint) Valid Florida driver's license and ability to operate private motor vehicle for routine travel Ability to work in large open office environment and maintain confidentiality #J-18808-Ljbffr Community and Social Services
