About the Position

Duration: 13 Weeks
Shift: Days | Monday–Friday | 8:00 AM – 4:30 PM

Responsibilities

Inpatient discharge planning
Coordinate patient discharges to home, SNF, community, or other facilities
Work alongside Social Workers and physicians
Manage approximately 17–25 patients
Attend interdisciplinary rounds
Make referrals for services such as dialysis, wound vacs, housing, shelters, and more

Requirements

Active RN License
BSN required
BLS Certification
Minimum 1 year of inpatient Case Management experience
PRI Certification
SCREEN Certification
Preferred: Critical Care experience and familiarity with CarePort, MCG, or InterQual
